Company History

Our Story

Community Partnerships delivered its first coordinated care service in 2000 as the provider of Children Come First (CCF) services for children and families living with serious mental health issues in Dane County. For several years, the CCF program was Community Partnerships’ only direct-service program in the organization. With dedicated strategic planning and a vision to offer high quality, strengths-driven coordinated care to the children, families, individuals that need them most, Community Partnerships has grown from a small organization of just over 20 staff to a leader in coordinated care in Dane County.

We celebrated our 15th anniversary in 2015 and now offer more than five community-based programs, serve more than 2,000 individuals annually, and employ more than 50 dedicated staff. Our services provide education, support, stabilization, and coordination of care to children, families, and individuals in our community.
