Physical Therapist

 

Community Care is hiring a Physical Therapist to join our Program team.   This rewarding opportunity is perfect for someone who enjoys team collaboration, community  case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity at our Milwaukee - Vliet facility.

 

Physical Therapist Responsibilities:

  • Functions as a member of care team(s) and participates in team meetings, including care planning, service/resource allocation, and updates on functional mobility.
  • Provides clinical care /service for an assigned caseload on a routine basis. Demonstrates exceptional evidence based clinical care delivery, conducts initial and annual assessments, develops member plan of care and goals, monitors member progress and updates care plan accordingly. Refers to and collaborates with other disciplines as needed.

 

Physical Therapist Requirements:

  • Education:Bachelor's degree in Physical Therapy is required. Masters degree in Physical Therapy preferred. Doctorate in Physical Therapy preferred
  • Experience: A minimum total of 1 year of personal, clinical/internship and or employment experience providing care or services for adults with disabilities or frail elderly population required.  Preferred: 5 years of personal, clinical/internship and/or employment experience providing care or services for adults with disabilities or frail elderly population required. Case Management experience
  • License:WI Physical Therapist license
  • Certification:CPR & AED

 

Rehabilitation Specialist Job Schedule:

  • Monday to Friday
  • 8:00AM – 4:30PM
